How to: Ban Request

When posting a ban request make sure you write down their name EXACTLY as it is spelled and if you don't have a name at least write down the time they were on.

Time logged on(GMT):

Reason for ban:

Proof: (Optional as it is hard to prove anything beyond screenshots in Minecraft unless you have something like Fraps or Hypercam)

Anything else:
